Order a birth certificate

This online form is to apply for a birth certificate for a person born in NSW or whose adoption was registered in NSW. If you have not yet registered the child's birth, you should complete the online birth registration form which will also allow you to order a birth certificate.

The type of certificate or search requested and the reason it is required (such as probate or estate administration, conveyancing or immigration, divorce proceedings).
The link between the client represented and the person whose name appears on the certificate.

False representation

If you knowingly provide false information in your application, you may be guilty of an offence under Section 57 of the Births, Deaths and Marriages Registration Act 1995.

A person who makes a representation in an application, notice or document under this Act or in response to a notice under Section 44 of this Act (Registrar's powers of inquiry), knowing the representation to be false or misleading in a material particular, is guilty of an offence.
